Recovery at Home Ireland services are responsible for the coordination and delivery for all services to public patient’s requiring IV antibiotics at Home Nationally. In addition to this we are also responsible for the the delivery of services to patients in our Virtual Ward and also support to patients who access our Healthcare Hub for pharma services. We also dispatch a range of medications as well as nursing interventions in response to over 10’000 calls each year and offer clinical support and medication. We are interested in seeking applications from Nursing staff with a background in pharma or specialist community service provision

JOB PURPOSE:

The Nurse should be registered in both Northern Ireland (UK) and the Republic of Ireland (ROI) will work in conjunction with the Operations Officer to ensure the safe and effective delivery of all Services RAHI operate in the community and lead with the business development of this fast-growing service. In conjunction with the CNM’s and Senior management team you will support and oversee the management of Patient Care on the Island of Ireland.

 

The team you work with will include Specialist Community Nursing Staff, Pharmacists Physiotherapists Health Care Assistances and associated staff.

 

To support productive community services and Pharma programmes, ensuring community nursing services and adopt efficient practices, proposing and implementing improvements where they are required to maintain outstanding operational and clinical effectiveness.

RESPONSIBILITIES:

  • Deliver an efficient and effective, community nursing service for patients, which is consistent with policies, procedures and good practice and which ensures receive seamless, evidence- based services.
  • Work closely with colleagues to ensure an integrated team culture, bringing together the different skills, experience and knowledge of team members to maximise the benefits of an integrated approach to service delivery for service users.
  • Develop operational services around Hospitals and GP Practices, and Pharma working with other professionals and staff, including physiotherapists, day services and any other staff relevant to the provision of an effective user-centred service.
  • Ensure that the team has the appropriate mechanisms to prioritise workload demands and allocate staff and resources appropriately, in accordance with assessed need and organisational policy.
  • Ensure that the assessment and provision of health care services is in accordance with access and eligibility criteria.
  • Ensure that effective record keeping practices, including use of IT systems, are maintained in accordance with organisational record keeping policies.
  • E.g. accuracy, confidentiality and timeliness.
  • Implement Adult Protection Procedures, in accordance with the Safeguarding Adults Guidance, chairing strategy meetings as appropriate.
  • Ensure that complaints are managed in accordance with the Complaints Procedure, and within the agreed time frames.
  • Ensure that all staff within the team undertake statutory and mandatory training.
  • Ensure effective communication processes are in place, with regular one-to- one meetings.
  • Monitor individual and/or team activity on a weekly basis to achieve the best use of resources and the designated key performance indicators

Skills:

Qualified Nurse minimum experience 4 years practice. (We are very keen to hear from nursing staff with a professional background in Pharma.)

 

Experience of delivering services in the community or on behalf of pharmaceutical industry.

JOB PURPOSE:

The Community Nurse manager will work in conjunction with the Chief Operations Officer to ensure the safe and effective delivery of all Services RAHI operate in the community and lead with the business development of this fast-growing service. In conjunction with the CNM’s and Senior management team you will oversee the management of teams of adult community nurses Nationally.

 

The teams will include Specialist Community Nursing Staff, Physiotherapists Health Care Assistances and associated staff.

 

To provide operational and clinical supervision, mentorship and coaching for community nurses ensuring that clinical supervision is in place for all community nurses.

 

To use the skills and experience gained as a senior practitioner to facilitate the operational, professional and clinical growth of teams and team members.

 

To support productive community services programmes, ensuring community nursing teams adopt efficient practices, proposing and implementing

improvements where they are required to maintain outstanding operational and clinical effectiveness.

 

To act as an ambassador for the service, developing robust communication strategies that provide all stakeholders with the opportunity to be engaged in service developments and informed on progress.

 

To ensure that the learning from incidents is acted upon and the learning shared throughout the community nursing teams.

 

To ensure that the requirements from national policy and local drivers is acted upon and implemented throughout the community nursing teams

MANAGERIAL RESPONSIBILITIES:

  • Ensure an appraisal process is in place so that all staff are appraised at least annually.

     

  •  Ensure that budgets under direct responsibility remain within set limits, implementing agreed monitoring and control systems for the use of agency and bank staff.

  •  Ensure effective communication processes are in place, with regular one-to- one meetings in place with direct reports and regular meeting with the wider team

  • Develop and monitor the teamwork /service plan in conjunction with the service/business planning process

     

  • Monitor individual and/or team activity on a weekly basis to achieve the best use of resources and the designated key performance indicators

Skills:

Qualified Nurse minimum experience 7 years practice, preferably 3 years of prior management experience. (We are very keen to hear from nursing staff with a professional background in Pharma.) Must be proficient in IT systems and able to communicate well by email. Experience of undertaking investigations and reporting findings both written and verbal.
