Accessing RAHI

Recovery at Home Ireland delivers healthcare in the community. We work with Ireland’s existing healthcare structures to provide high quality nurse led healthcare and social care at all times. Our care models reduce unnecessary hospital admissions or readmissions and can provide for even the most complex care requirements outside of the acute hospital.

Your RAHI Health

As part of the Recovery at Home Ireland commitment, every patient will have a named ‘Recovery at Home Ireland’ Health Carer who will be their personal champion throughout the time spent as a HNP patient.


From your initial contact, you or your family will be contacted a Recovery At Home Health representative who will arrange a visit with you to discuss our services and your individual needs.

How To Get Started

We will then arrange a complete assessment visit. As part of our assessment visit, your Recovery at Home Ireland Health Carer will complete an assessment of your needs with you from this complete Individual Care Plan specific to your requirements. (We are also able to provide services to children under the age of 18) 

Choosing the Right Team

We will provide you with the most appropriate team which may include a:

  • Doctor or Geriatrician

  • Occupational Therapist

  • Physiotherapist

  • Speech and Language Therapist

  • Specialist Nurse

  • Psychologist

  • Super Carer

  • Personal Nursing Assistants

  • Personal Support Assistant

This will be based on your specific requirements and who will work with you according to your Individual Care Plan.
We take a collaborative approach to your needs, including family, friends and the multidisciplinary team as appropriate, in completing your Individual Care Plan.
